Amit Koushal of Uttar Pradesh, an officer working at the Seabird naval base in Karwar, his wife and child were killed in a road accident in 2018. Not satisfied with the paltry amount paid by Bharti AXA Insurance Company, Amit's mother, Pushpadevi, had filed a case in the principal district and sessions judge court. The court had decreed that the insurance company pay her Rs 15 lakh.

In October 2019, Pushpavati moved the Dharwad bench of the high court questioning the decree of the district court and seeking higher compensation. She was represented by advocate Girish Hulmani while advocate SK Kantimath represented the insurance company. The matter was referred to the Lok Adalat and the court decreed the company should pay Rs 74 lakh to her.
